Your Care Plan
A Senior Registered Nurse will draw up an individual nursing care plan reviewing your needs and prescribing the nursing care you should be receiving. You and your relatives can be involved in this process and it will be reviewed monthly or more frequently if there are changes in your condition.
Reviews of your care plan are carried out on a monthly basis. Reviews are more frequent if there are changes in your condition making it necessary to indicate a change to the level of the care you require.
